Clinical Manager

Key Responsibilities:

• Lead and mentor nurses, LPNs, and caregivers with professionalism and empathy.

• Oversee internal case management and ensure the delivery of client-centered, compliant care.

• Conduct admissions, client assessments, care planning, and clinical documentation.

• Serve as an internal case manager, advocating for clients and collaborating with caregivers and physicians.

• Maintain strong communication with team members, MCOs, and insurance case managers.

• Participate in incident investigations, infection control oversight, and quality improvement activities.

• Observe and evaluate field staff performance through chart review and in-home supervision.

• Provide ongoing education and support to team members, ensuring safe, effective, and compassionate care.

• Contribute to hiring, orientation, skill evaluation, and staff development efforts.

• Maintain accurate, timely documentation and uphold all regulatory standards.

Qualifications:

• Current Registered Nurse (RN) license in good standing in [Insert State].

• BSN preferred; Associate Degree in Nursing required.

• Minimum 2 years of recent clinical experience, with at least 1 year in home health or supervisory experience preferred.

• Demonstrated ability to lead, coach, and evaluate nursing and caregiving staff.

• Strong clinical assessment and decision-making skills.

• Excellent written and verbal communication skills.

• Detail-oriented, with excellent organizational and time management abilities.

• Competency in basic computer skills and EMR systems.

• Reliable transportation and willingness to travel for field visits.
