Key Responsibilities:
Lead and mentor nurses, LPNs, and caregivers with professionalism and empathy.
Oversee internal case management and ensure the delivery of client-centered, compliant care.
Conduct admissions, client assessments, care planning, and clinical documentation.
Serve as an internal case manager, advocating for clients and collaborating with caregivers and physicians.
Maintain strong communication with team members, MCOs, and insurance case managers.
Participate in incident investigations, infection control oversight, and quality improvement activities.
Observe and evaluate field staff performance through chart review and in-home supervision.

Qualifications:
Current Registered Nurse (RN) license in good standing in [Insert State].

Primary Responsibilities:
Deliver personalized nursing care according to the client's individualized care plan and the physician’s Plan of Treatment.
Administer prescribed medications, treatments, and procedures.
Monitor and record client symptoms, changes in condition, and progress toward goals.
Promptly report condition changes and execute new physician orders.
Update and revise care plans as necessary to reflect changing needs.
Maintain accurate, timely documentation in accordance with state regulations and agency standards.
Follow all infection control and safety protocols.
Perform clinical tasks based on competency, such as:
Glucometer and PT/INR testing
Tracheostomy and ventilator care (as applicable)

Minimum Qualifications:
Current, unrestricted LPN license in the state of practice.
At least one year of verifiable LPN experience (Home Health preferred).

Successful completion of written competency exams.
