Responsibilities:

● Perform in-home and ensure visit time fully meets the needs of the patient.

● Coordinate total patient care, including interdisciplinary communication with other health care providers to enhance continuity of care.

● Render skilled nursing care, as assigned by the registered nurse, according to the physician’s orders.

● Recognize and interpret symptoms; report patient’s condition and assist with or institute remedial measures for adverse developments.

● Guide and instruct patient and their families in prescribed activities that are directed toward improving independence and functionality.

● Ensure lab tests are obtained, stored, and transported according to acceptable laboratory practice and OSHA guidelines.

● Review and update the medication profile; assess patient compliance, understanding of medication regime, contraindications, and side effects.

Qualifications

● Must be an LPN or LVN in the state of employment.

● Must have at least one year of experience as an LPN or LVN.

● Medicare home health experience is preferred.

CNA/HHA Requirements:

Our PRN Registered Nurse (RN) administers skilled care visits to clients requiring intermittent professional services and teaches the client, family, and other members of the healthcare team. These

services are performed in accordance with the physician’s orders and the established plan of care under the direction and supervision of the Branch Director.

Qualifications

1. Must hold a current and unencumbered Registered Nursing License in the USVI

2. Must be organized, detail-oriented, and possess practical communication skills both orally and in writing.

3. RN must understand the issues related to the delivery of home health care and be able to problem-solve effectively.

4. Ability to assume a flexible work schedule.
