Registered Nurse (RN) - Home Health - PRN

As a Registered Nurse (RN), you will be instrumental in providing individualized, skilled nursing care to clients in their homes. You will assess health needs, develop customized care plans, and support patients in reaching their highest potential for health, comfort, and independence.

Key Responsibilities:

• Conduct thorough assessments and develop individualized plans of care.

• Deliver professional nursing services in line with physician orders and care plans.

• Monitor and document client status, condition changes, and treatment outcomes in the EMR system within 24 hours.

• Educate clients and families on self-care strategies and wellness.

• Supervise LPNs or Home Health Aides as needed.

• Participate in quality assessment activities, staff education programs, and interdisciplinary team meetings.

• Provide care that reflects the diverse living situations, family dynamics, and preferences of each client.

• Maintain confidentiality and uphold all state/federal regulations, agency policies, and infection control protocols.

• Communicate effectively with clients, families, and the care team.

• Travel to assigned client homes.

Required Qualifications:

• Active and unrestricted RN license in [insert state]
